If you are a new patient, please fill out the registration forms listed below in advance of your appointment to assist the staff in making sure that we have all the information necessary to provide you with quality care and treatment. It is helpful if you bring along copies of previous tests, lab results and the names of any medications or natural supplements you are currently taking. Please bring the completed forms with you along with your ID to the appointment.
Getting Ready for Your Visit
In order for us to provide you with fast, quality care, please bring the following items to your visit:
Photo ID (required for credit card, check or insurance verification only).
Insurance information (if you have insurance).
Copay, deductible or self-pay amounts (due at the time of treatment).
List of any current medications.
List of any past medical conditions and allergies.
Dr. Silva’s Ultra Wellness Center accepts all major insurance plans, including most products from the carriers listed below. Individual plans may differ and we recommend you consult your health insurance provider for detailed coverage information. Depending on your plan, you may be responsible for co-payments, co-insurance or deductibles. If your plan is not currently accepted, Dr. Silva’s Ultra Wellness Center provides affordable self-pay rates.
